La’Verne Webb/Soul2SoleBounceFitness, LLC, advises you to obtain a physical examination
from a doctor before using any exercise equipment or participating in any exercise activity.
You agree that by participating in physical exercise with Soul2Solebouncefitness,LLC/La’Verne
Webb, you do so entirely at your own risk. You agree that you are voluntarily participating in these
activities and assume all risks of injury, illness, or death. Soul2SoleBounceFitness, LLC/La’Verne
Webb is not responsible for any loss of your personal property. You acknowledge that you have
carefully read this “waiver and release” and fully understand that it is a release of liability. You
expressly agree to release and discharge Soul2SoleBounceFitness, LLC/La’Verne Webb from any
and all claims or causes of action and you agree to voluntarily give up or waive any right that you
may otherwise have to bring a legal action against the trainer or instructor for personal injury or
property damage. To the extent that statute or case law does not prohibit releases for negligence,
this release is also for negligence. If any portion of this release from liability shall be deemed by a
Court of competent jurisdiction to be invalid, then the remainder of this release from liability shall
remain in full force and effect and the offending provision or provisions severed here from.
Assumption of the Risk and Waiver of Liability Relating to Coronavirus/COVID-19: The
coronavirus (COVID-19), has been declared a worldwide pandemic by the World Health
Organization. COVID-19 is extremely contagious and can spread from person to-person
contact. Soul2SoleBounceFitness/La’Verne Webb has and will continue to use best practices
to institute and implement preventative measures to reduce the spread of COVID-19;
however, cannot guarantee that you may not become infected, exposed or otherwise
contract COVID-19 while attending, participating in or otherwise engaging in any activities
at or in connection with Soul2solebouncefitness/La’Verne Webb
By signing this waiver and release, I acknowledge and agree that I: Understand the
contagious nature of COVID-19, voluntarily assume the risk that I may become infected,
exposed or otherwise contract COVID-19 while attending, participating in or otherwise
engaging in any activities at or in connection with Soul2solebouncefitness, LLC/La’Verne
Webb and hereby waive, release, and discharge Soul2SoleBounceFitness, LLC/La’Verne
Webb from and against any and all claims or injuries arising out of, relating to or in any way
connected to COVID-19 and the subject of this Waiver and Release.

Soul2SoleBounceFitness, LLC
